Shift & Schedule

  • Night shift: 4x10-hour shifts
  • Working hours: 16:00 to 2:00

Requirements & Skills

  • Minimum of 3 years of professional experience in social services
  • Active licensure as LCSW, LMFT, or LMHC
  • Proficiency with Epic charting system (required)
  • Experience with psychiatric hospital protocols, including 5150/involuntary holds and completing intake and admission screenings
  • Ability to perform comprehensive bio-psychosocial assessments
  • Provide patient and family education, support, and advocacy
  • Skilled in working collaboratively within a multi-disciplinary team to ensure continuity of care
  • Knowledge and experience related to adoption/foster care, drug/alcohol treatment options, suicide/homicide/self-harm, and protective services including adult and child protective services, emergency detention, guardianship, and power of attorney
  • Competency in assessing patient response to treatment, conducting initial evaluations, and biopsychosocial assessments
  • Experience in case management including care coordination and utilization review
  • Proven ability to conduct crisis and mental health assessments, crisis intervention for teen suicides, substance abuse, and school violence
  • Strong understanding of trauma-related concerns and ethical clinical principles
  • Established knowledge regarding dementia, psychiatric disorders, and emergency/psychiatric department protocols
  • Cultural competence and awareness of community programs such as Medicaid and Medicare
  • Expertise in facilitating discharge planning including patient/caregiver education and crisis intervention

Key Responsibilities

  • Complete intake and admission screenings and conduct thorough biopsychosocial assessments
  • Provide education, support, and advocacy to patients and their families, ensuring they understand treatment plans and resources
  • Collaborate with a multidisciplinary team to maintain effective and coordinated care throughout the patient’s treatment journey
  • Carry out crisis interventions addressing suicide risk, substance abuse, and behavioral emergencies within school or psychiatric settings
  • Evaluate and document patient progress and response to treatment, facilitating timely modifications to care plans as needed
  • Manage cases with careful coordination of internal and external resources, ensuring continuity and comprehensiveness of care
  • Maintain accurate and detailed documentation in the Epic charting system to support care coordination efforts
  • Identify and address trauma-related, social, emotional, and behavioral needs using evidence-based social work principles
  • Participate actively in discharge planning and contingency planning to support patient transitions from acute care settings

Qualifications

  • Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Mental Health Counselor (LMHC) required
  • Minimum three years of relevant clinical experience in psychiatric or social services settings
  • Experience using Epic for patient charting and documentation
  • Strong knowledge of adult and child protective services and emergency mental health holds (5150)
  • Well-versed in crisis intervention techniques and development of clinical assessment reports
  • Demonstrated ability to work effectively with culturally diverse populations and community programs such as Medicaid and Medicare
  • Excellent communication, interpersonal, and organizational skills
  • Commitment to ethical principles in healthcare social work and patient advocacy
